چکیده

C lozapine is an atypical antipsychotic that is effective in treatment resistant schizophrenia. The National Institute for Health and Clinical Excellence (NICE) guidelines for schizophrenia specify that ‘‘in individuals with evidence of treatment resistant schizophrenia, clozapine should be introduced at the earliest opportunity’’. A severe adverse effect of clozapine that limits its more widespread use is agranulocytosis. Patients who are taking clozapine need to have their full blood counts (FBC) monitored regularly, and if the total white cell and/or neutrophil counts indicate agranulocytosis, clozapine prescription must be terminated. Among certain ethnic groups, a significant proportion of people have a low baseline neutrophil count. This is called benign ethnic neutropenia (BEN). This editorial looks at the important issues associated with agranulocytosis and BEN in patients receiving clozapine.

Neutropenia and agranulocytosis during treatment of schizophrenia with clozapine versus other antipsychotics: an observational study in Iceland

BACKGROUND Data on the haematological outcomes of patients who continue clozapine treatment following neutropenia are very rare as even mild neutropenia results in mandatory discontinuation of clozapine in most countries. However, in Iceland where clozapine monitoring is less stringent allows an observational study to be done on the risk of agranulocytosis and neutropenia during treatment with ...

Comment on "lithium and clozapine rechallenge: a retrospective case analysis".

Sir: We refer to the interesting article from Drs. Kanaan and Kerwin 1 concerning the adjunctive use of lithium to treat clozapine-induced neutropenia. Immediate discontinuation of clozapine treatment has always been the first therapeutic step in cases of severe to moderate neutropenia and impending agranulocytosis. However, some physicians are reluctant to terminate or even to interrupt cloza-...

Clozapine-Induced Febrile Neutropenia and Cellulitis

Clozapine is one of the atypical antipsychotics and is frequently prescribed to patients with treatment-resistant schizophrenia. Agranulocytosis is a major side effect that may lead to death, which limits its use. This is a case report of a patient that developed febrile neutropenia and cellulitis after treatment with clozapine for 20 weeks.

Pancytopenia on switching from clozapine to olanzapine: A case report and some unresolved issues

The newer antipsychotics olanzapine and clozapine share a number of structural and functional similarities. However, unlike clozapine, which induces agranulocytosis in about 1% of treated patients, olanzapine is not normally known to cause hematological adverse effects. The hematological effects of olanzapine have been scrutinized in several trials, none of which have found any evidence of hemo...

BRIEF REPORT Clozapine-Associated Agranulocytosis Treatment With Granulocyte Colony-Stimulating Factor/Granulocyte- Macrophage Colony-Stimulating Factor A Systematic Review

Purpose/Background: Clozapine is associated with hematological abnormalities, notably neutropenia, which may progress to agranulocytosis. Granulocyte colony-stimulating factor (G-CSF) and granulocyte-macrophage colony-stimulating factor (GM-CSF) have been used to reduce the frequency and duration of clozapine-associated neutropenia. This review aims to explore the use, efficacy, and tolerabilit...
